The Occupational Therapist (OT) is responsible for the daily management of an assigned patient caseload in a home health setting. This role ensures timely care and documentation to meet patient needs and services as prescribed in the established plan of care and per physician orders. The OT evaluates the patient's functional ability to perform activities of daily living, assesses their potential for achievement, and develops a plan of care in conjunction with the patient's physician. They design, develop, and fit patients with orthotic devices and adaptive equipment as needed, and instruct family members, home health aides, and other clinical staff in appropriate therapy methods. The OT also observes, records, and reports any reactions to treatment or changes in patient condition to the physician. This position requires maintaining communication with the multidisciplinary team, incorporating short and long-term goals into therapy, and supporting effective communication with physician oversight and branch supervision. The role involves providing clear, timely, and complete documentation, adhering to productivity standards, and performing ordered treatments in accordance with state and federal regulations, JCAHO, Medicare Guidelines, OASIS, and agency policies. The OT delegates elements of care to appropriate healthcare workers with minimal supervision and promotes shared governance, research, and effective communication. They are expected to attain and maintain current occupational therapy practice knowledge, integrate evidence and research findings, contribute to quality practice, and demonstrate leadership.
