About The Position

Elderwood Skilled Nursing & Rehabilitation in Amherst, NY is seeking a compassionate and clinically driven Occupational Therapist (OT) to join their skilled nursing and rehabilitation team. This role offers the opportunity to practice patient-centered therapy in a collaborative environment where OTs are valued for their clinical insight, creativity, and ability to improve quality of life through functional recovery and meaningful daily activity. The position focuses on helping older adults maximize independence, restore function, and safely navigate daily living. Elderwood emphasizes functional, meaningful therapy, a collaborative clinical environment with PTs, SLPs, Nursing, and providers, and a diverse clinical population including orthopedic recovery, neurological conditions, stroke recovery, cognitive and dementia-related challenges, ADL retraining, balance and safety concerns, and long-term maintenance therapy needs. They provide supportive operations with strong communication, scheduling, and interdisciplinary collaboration.

Requirements

  • Graduation from an approved school of occupational therapy required.
  • State license to practice occupational therapy required.
  • Current, valid CPR/BLS credential
  • Ability to communicate well verbally and in writing required.
  • Sense of responsibility and ability to work well with residents, families, and other staff members required.
  • Qualities of maturity, diplomacy, and ability to work well with others required.
  • Ability to work with geriatric and chronically ill residents, as well as other professionals in a friendly and helpful manner necessary.
  • Ability to perform tasks to established standards of excellence required.
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.

Nice To Haves

  • Two years’ experience in occupational therapy preferred.
  • Interest in occupational therapy treatment programs for the geriatric and subacute populations utilizing a team approach.
  • Ability to direct others necessary.

Responsibilities

  • Provide skilled occupational therapy evaluations and treatments in accordance with physician orders
  • Develop and update individualized treatment plans and care goals
  • Maintain timely and accurate clinical documentation
  • Collaborate with interdisciplinary teams regarding resident progress and discharge planning
  • Participate in resident care planning meetings
  • Support resident and family education initiatives
  • Assist with mentorship and clinical support activities, as needed
