The occupational therapist contracted through the Organization is responsible for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the Service Agreement. This role involves the ongoing interdisciplinary assessment and development of individualized plans of care, providing physician-ordered services, patient/caregiver counseling and education, and communication with physicians and other healthcare practitioners. The therapist will evaluate patient's functional status, home environment, and equipment needs, participating in the total plan of care. For patients returning to work, the therapist may perform work capacity evaluations and refer to vocational training. Treatment programs will be developed with goals for improved function, and may include teaching new skills or retraining in daily activities, fabricating splints, and instructing in the use of adaptive, prosthetic, and/or orthotic devices. The role also requires maintaining clinical records, participating in quality improvement programs, identifying needs for other home health services, instructing families and personnel, supervising Certified Occupational Therapy Assistants and Home Health Aides, and participating in discharge planning.
