About The Position

All Heart Home Care is seeking a compassionate and skilled Occupational Therapist (Full-Time) to join our home health team in the Murfreesboro area. This role offers a consistent schedule while providing personalized, one-on-one care to patients in the comfort of their homes.

Requirements

• Degree in Occupational Therapy from an accredited program
• Active Occupational Therapist license in the state of Tennessee
• Current CPR certification
• Valid driver's license, reliable transportation, and current auto insurance
• Strong clinical assessment and communication skills
• Ability to work independently and manage time effectively

Nice To Haves

• Previous home health experience preferred but not required

Responsibilities

• Perform in-home evaluations to assess patient needs and develop individualized treatment plans
• Provide skilled occupational therapy services to improve daily living skills, strength, coordination, and independence
• Monitor patient progress and adjust treatment plans as appropriate
• Educate patients and caregivers on adaptive techniques, home safety, and therapeutic exercises
• Complete accurate and timely clinical documentation in compliance with company policies and state regulations
• Coordinate care with physicians, nurses, case managers, and other members of the healthcare team
• Travel to patient homes within assigned service areas
