About The Position

Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Occupational Therapist to provide in-home care. This is an opportunity for an Occupational Therapist ready to take the next step forward in their career to join a growing organization within a progressive, supportive, and innovative team. All levels of experience are encouraged to apply, and training will be provided for the right new graduate. The role involves providing 1:1 treatment to patients within a designated geographic area, developing individualized treatment plans, and delivering care that aligns with professional standards. The position also includes educating caregivers and documenting patient progress.

Requirements

  • Occupational Therapist Program Graduate
  • Licensed Occupational Therapist in the state of New York or eligible.
  • Ability to travel to patients’ homes within the assigned geographic territory
  • Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.

Nice To Haves

  • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.

Responsibilities

  • Accept and treat patients within your designated geographic area
  • Provide 1:1 treatment
  • Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
  • Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
  • Educate family members, home health aides, and other caregivers on proper therapy techniques
  • Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
  • Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
  • Provide timely and accurate documentation for each visit
  • Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
  • Must be willing to treat geriatric patients.
