Occupational Therapist (OT) Per Diem - Home Care
About The Position
Lehigh Valley Health Network (LVHN) is seeking an Occupational Therapist (OT) for a Per Diem Home Care position. This role involves evaluating, planning, directing, and administering Occupational Therapy programs to help patients restore function, prevent disability, and achieve maximum independence. The OT will assess patient conditions based on physician diagnoses, develop care plans in collaboration with patients and families, and facilitate participation in daily living activities to improve quality of life. The position requires a commitment to providing excellent healthcare within a nationally recognized health network known for its teamwork, compassion, and technology.
Requirements
- Bachelor’s Degree in Occupational Therapy
- Less than 1 year of experience
- Successfully completed clinical experiences as required by the school program.
- Demonstrates sound clinical reasoning in synthesizing data gathered in the evaluation and identifying treatment diagnoses.
- Ability to work at various locations per diem.
- Knowledge of occupational therapy techniques.
- Must be able to communicate with patients, family, and co-workers in a friendly, supportive manner.
- Must be able to provide safe, compassionate, and competent occupational therapy treatments.
- Ability to travel to patient care assignments - Home Care only
- American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
- OT - Licensed Occupational Therapist - State of Pennsylvania Upon Hire
Nice To Haves
- Master’s Degree in Occupational Therapy or Doctorate in Occupational Therapy
- 1 year Clinical experience required for Home Care.
Responsibilities
- Establishes and implements appropriate occupational therapy plans of care to restore function, prevent disability, and help patients reach their maximum performance level.
- Implements an individualized treatment plan using skilled evidence-based therapy techniques such as therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities.
- Provides occupational therapy treatment consistent with the patient's needs assessment.
- Performs reassessment when indicated and modifies occupational therapy plans accordingly.
- Communicates patient progress and physical limitations to a supervisor, physician, and other team members.
- Implements an occupational therapy plan of care to individuals based on age-specific and population-specific needs utilizing a developmental and patient family-directed approach.
- Provides supervision in accordance with regulatory requirements, as appropriate.
- Establishes an occupational therapy treatment plan with measurable goals and time frames.
- Provides cost-effective care in accordance with departmental billing guidelines.
- Completes and submits all documentation based on policies/standards and regulatory requirements.
