About The Position

The Occupational Therapist (Per Diem/PRN) is responsible for providing high-quality, patient-centered therapy services in the home setting. The OT evaluates, plans, and implements individualized treatment programs to improve patients’ ability to perform daily activities, promote safety, and enhance overall well-being. This position works collaboratively with the interdisciplinary home health team to ensure effective, coordinated care.

Requirements

  • Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.
  • Current and valid Occupational Therapy license in the State of California.
  • Current CPR certification.
  • Valid driver’s license and reliable transportation.
  • Proof of automobile insurance.

Nice To Haves

  • Minimum of one (1) year of occupational therapy experience preferred.
  • Home health experience strongly desired.
  • Strong organizational and time-management skills for per diem scheduling.
  • Comprehensive understanding of home health regulations and documentation standards (OASIS familiarity preferred).
  • Excellent assessment, communication, and patient education skills.
  • Compassionate, patient-centered approach to care.
  • Ability to work independently and manage a flexible caseload.
  • Proficiency with electronic medical record (EMR) systems, Axxess preferred.
  • Commitment to maintaining clinical excellence and compliance standards.

Responsibilities

  • Conduct comprehensive assessments to evaluate functional abilities, home environment, and rehabilitation potential.
  • Develop individualized care plans and treatment goals that address physical, cognitive, and psychosocial needs.
  • Provide therapeutic interventions to improve strength, coordination, fine motor skills, and independence in activities of daily living (ADLs).
  • Educate patients and caregivers on adaptive techniques, safety strategies, energy conservation, and use of assistive devices or equipment.
  • Recommend and coordinate durable medical equipment (DME) and environmental modifications as needed.
  • Document all evaluations, treatments, and progress notes in compliance with Wellpointe Home Health policies, state, and federal regulations.
  • Collaborate with nurses, physical therapists, speech therapists, and medical social workers to ensure a cohesive plan of care.
  • Participate in case conferences, team meetings, and quality improvement initiatives as requested.
  • Communicate effectively with physicians and care coordinators regarding patient progress and needs.
