About The Position

Adaptive is building an AI-native home healthcare platform and expanding its presence in Detroit. The Occupational Therapist delivers excellent home health care to patients in Detroit while helping establish the clinical standards, documentation practices, and referral-source trust that will support the market’s growth. The role involves completing in-home OT evaluations and skilled visits, developing plans of care focused on ADLs/IADLs, safety, fall prevention, and functional independence, and recommending and training patients on adaptive equipment and home modifications. This is a field-based, part-time flexible PRN role. The therapist picks up visits based on availability, on weekdays and/or weekends as needed, and travels throughout an assigned territory. The clinician is expected to take ownership of patient outcomes and their caseload, contribute to quality improvement through case conferences and team huddles, build strong relationships with referral partners, and help shape the team’s playbook and standard operating procedures.
