About The Position
We’re hiring an Occupational Therapist to serve patients in the Rochelle area, helping individuals regain independence and safely perform daily activities in their home environment. Join Castor Home Health Solutions as an Occupational Therapist (OT) and provide one-on-one care in patients’ homes without the stress of back-to-back appointments or facility demands.
Requirements
- Active Illinois Occupational Therapist (OT) license
- Strong clinical and assessment skills
- Compassionate, patient-focused approach
- Reliable transportation

- Home health experience preferred
Responsibilities
- Perform in-home occupational therapy evaluations
- Develop individualized treatment plans focused on ADLs/IADLs and independence
- Provide one-on-one therapy sessions
- Educate patients and caregivers on safety, mobility, and adaptive techniques
- Recommend and train on adaptive equipment
- Collaborate with interdisciplinary care teams
- Document progress and outcomes
