About The Position

Living Hope Home HealthCare is seeking a Licensed Occupational Therapist (OT) to join their team in the Houston, TX area and surrounding areas. The OT will provide coordinated care to patients, evaluate their needs, and work to restore or maintain their well-being according to the plan of care. This role involves evaluating patients with physical, psycho-social, and/or cognitive impairments, developing treatment plans, and providing interventions to address deficits. The OT will also collaborate with an interdisciplinary team.

Requirements

  • Bachelor of Science in Occupational Therapy
  • Current Texas State License as an Occupational Therapist
  • 1 year of minimum experience as an Occupational Therapist in Home Healthcare or Acute care or Rehabilitation setting
  • Therapy skills as defined as generally accepted standards of practice.
  • Good communication and interpersonal skills.
  • Proof of current CPR and Hepatitis profile.
  • Knowledge of durable medical equipment
  • Occupational Therapy License (Required)
  • CPR (Required)

Nice To Haves

  • Spanish is a Plus
  • Home health: 1 year (Preferred)

Responsibilities

  • Perform initial and ongoing clinical assessments to identify patient needs, determine level of function at appropriate time points and the appropriateness of the patient for Home health.
  • Develop and revise a plan of care in consultation with the physician and other care team members.
  • Provide therapeutic treatments, evaluate need for and use of adaptive equipment, and environmental modifications, fabricate and maintain adaptive equipment and hand splints as applicable.
  • Evaluate outcomes of care with appropriate discharge planning.
  • Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
  • Accept clinical assessments that are consistent with education and competence to care for patients.
  • Supervise COTA as needed.
  • Complete appropriate documentation in a timely manner.
  • Meet the mandatory continuing education requirements of the agency/license board.
  • Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate.
  • Demonstrate commitment, professional growth and competency.
  • Promote Agency philosophy and administrative policies to ensure quality of care.
