About The Position
Ultimate Therapy is seeking a full-time Occupational Therapist to deliver hands-on, patient-centered care in a skilled nursing facility. This role emphasizes restoring independence, improving functional ability, and enhancing overall quality of life for residents.
Requirements
- Active Occupational Therapist license required

Responsibilities
- Conduct comprehensive assessments and establish goal-driven care plans
- Provide skilled interventions addressing self-care, cognition, and daily function
- Introduce adaptive strategies and recommend assistive devices to support independence
- Reassess resident performance and modify treatment approaches accordingly
- Partner with nursing, therapy staff, and families to coordinate care
- Ensure timely and accurate clinical documentation
