We’re seeking a compassionate, motivated Occupational Therapist (OT) to join our team at a beautiful, modern Skilled Nursing Facility with Assisted Living in Brooksville, FL. This is your chance to be part of a positive transformation, working in a setting designed to support both patients and clinicians. Therapy services are provided by Creative Health Solutions, an in-house therapy company that prioritizes quality over quotas —giving you the time, tools, and collaboration you need to deliver meaningful, patient-centered care. CHS is offering a $10,000 sign-on bonus for this position.

Valid State OT License

Responsibilities

Help residents regain independence and improve quality of life
Deliver individualized, evidence-based therapy
Collaborate closely with nursing and leadership teams
Be a key part of a facility that’s actively evolving and improving
Develop and administer therapy programs aimed at improving patients' physical, mental, and emotional well-being.
Work closely with a team of healthcare professionals to create personalized treatment plans for patients.
Document patient progress and communicate with healthcare professionals regarding treatment plans and patient outcomes.
Continuously educate patients and their families on therapeutic procedures and techniques
Stay up to date with the latest therapy techniques and methodologies.
