About The Position

City View Post Acute is seeking a compassionate, motivated, and dedicated Occupational Therapist (OT) to join our exceptional in-house rehabilitation team. We welcome both experienced Occupational Therapists and new graduates, offering a supportive environment with mentorship, professional growth, and a collaborative team dedicated to providing excellent resident care. The role involves evaluating residents, developing individualized treatment plans, and providing skilled interventions to improve function, independence, and quality of life.

Requirements

  • Current California Occupational Therapist (OT) license or eligibility for licensure.
  • Strong clinical, communication, and problem-solving skills.
  • Excellent interpersonal and organizational skills.
  • Passion for rehabilitation and resident-centered care.
  • Positive, collaborative, and compassionate approach to patient care.
  • Strong documentation and time-management skills.

Responsibilities

  • Perform comprehensive occupational therapy evaluations and assessments.
  • Develop individualized treatment plans based on resident needs, goals, and functional abilities.
  • Provide skilled therapeutic interventions to improve activities of daily living, strength, coordination, balance, mobility, and functional independence.
  • Address cognitive, physical, and functional limitations that impact residents' ability to participate in daily activities.
  • Provide training in adaptive techniques, energy conservation, safety strategies, and use of adaptive equipment.
  • Educate residents and families regarding therapeutic exercises, safety, functional mobility, and strategies for increasing independence.
  • Monitor resident progress and modify treatment plans as appropriate to maximize outcomes.
  • Collaborate with physicians, nursing staff, Physical Therapists, Speech Therapists, and the interdisciplinary care team.
  • Accurately document evaluations, treatments, progress notes, and discharge planning.
  • Participate in care planning and interdisciplinary meetings as appropriate.
  • Maintain a compassionate, professional, and resident-centered approach to care.
