About The Position
We are seeking a passionate Occupational Therapist (OT) to join our team for home health visits in Lake County. This role offers a flexible schedule and the opportunity to build meaningful patient relationships. You will perform occupational therapy evaluations, develop individualized treatment plans to improve patient independence and safety, assess ADLs and functional limitations, educate caregivers and patients, and coordinate care with interdisciplinary teams.
Requirements
- Active Florida OT License

- Home health experience preferred
- HCHB experience preferred
- Strong clinical reasoning skills
