About The Position

SummitWest Care, a local nonprofit home health agency, is seeking a compassionate Occupational Therapist (OT) to provide one-on-one care throughout Mesa County. This role offers a unique therapy experience focused on helping patients safely and confidently navigate everyday life in their own homes, emphasizing meaningful relationships and tailored treatment plans. The position provides flexibility and autonomy in the field, supported by an experienced interdisciplinary team dedicated to both patient outcomes and clinician success. Home health allows for focused patient care, enabling education, problem-solving, and connection, with therapy having the greatest impact in the patient's real-life environment. Requirements

  • Current Colorado Occupational Therapist (OT) license in good standing
  • Graduate of an accredited Occupational Therapy program
  • Strong clinical assessment, communication, and documentation skills
  • Ability to work independently in a home health environment
  • Excellent time-management and organizational skills
  • Valid driver's license, reliable transportation, and auto insurance

Nice To Haves

  • Home health experience is a bonus, but not required!

Responsibilities

  • Perform occupational therapy evaluations, reassessments, recertifications, and discharges
  • Develop and implement individualized treatment plans focused on safety, function, and independence
  • Assess patients' ability to perform ADLs
  • Conduct home safety evaluations and recommend adaptive equipment or environmental modifications
  • Educate patients and caregivers on home programs, mobility strategies, and safety techniques
  • Monitor patient progress and adjust treatment plans as appropriate
  • Collaborate with nurses, therapists, caregivers, physicians, and family
  • Supervise and support COTAs when applicable
  • Complete accurate and timely clinical documentation
