About The Position

Occupational Therapist - Home Care - Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the general territory for this position would be the Southwest Twin Cities Metro area. This includes the cities that are south of Highway 394 and west of 35W. Requirements

  • Current license in the State of Minnesota as an Occupational Therapist (OT).
  • Two (2) years of experience as an Occupational Therapist.
  • Able to travel within geographic area serviced by the office.
  • Valid driver's license, reliable transportation, and proof of auto insurance required.
  • CPR Certification

Responsibilities

  • Completes initial and ongoing comprehensive assessments at appropriate time points as assigned.
  • Participates in ongoing interdisciplinary assessment of the patient.
  • Provides services that are ordered by the physician as indicated in the plan of care.
  • Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
