About The Position

Fairview Range is looking for a Occupational Therapist to join our Hibbing Home Care and Hospice team. We take great pride in the privilege of providing compassionate care to our patients and their families while serving communities throughout the expansive Iron Range. As a dedicated Occupational Therapist, you coordinate the delivery of occupational therapy services tailored to the unique needs of clients spanning all age groups. Our approach involves meticulously crafting a comprehensive care plan, encompassing essential education and support for both clients and caregivers, continual progress monitoring, hands-on care provision, and resource management. Additionally, we provide invaluable supervision to paraprofessional staff and maintain open collaboration with the Supervisor/Manager to address pertinent issues.

Requirements

  • Bachelor of Arts in Occupational Therapy
  • Graduation from accredited school of Occupational Therapy
  • Maintains continuing education per state regulations
  • Less than 1-year acute care, long term care or clinic experience
  • Basic Life Support (American Heart Assoc or Red Cross)
  • Occupational Therapist Reg MN Upon Hire

Responsibilities

  • Ongoing development and implementation of the plan of care
  • Maintaining an individualized plan of care, related to the client's needs, in collaboration with the physician, client and caregiver.
  • Continually reassessing the client's health status, environment, and support system with ongoing comprehensive assessment. Plans maximize independence of client/caregiver.
  • Continually reassessing resource utilization as client progresses towards goals.
  • Organizing/Staffing/Directing the coordination of services
  • Accountable for managing the clinical episode of care to achieve desired client outcomes and measurable functional goals.
  • Communicating consistently and effectively with client, caregiver, physicians, and all team members to assure appropriate implementation and completion of the client plan of care.
  • Managing client expectations.
  • Advancing the POC with the multidisciplinary team. Consults with the other team members as needed to make modifications in the plan. Decreases utilization of services as client moves towards goal achievement.
  • Educates paraprofessional staff on the client's plan of care.
  • Providing safe, effective skilled and advanced intervention, within scope of practice according to client need.
  • Providing counseling and teaching of client, family and caregiver regarding care needs and health promotion/prevention.
  • Evaluating and documenting response to care/services delivered.
  • Demonstrating ability to adjust approaches to care to reflect developmental level and cultural differences of populations served.
