About The Position

Metro Physical & Aquatic Therapy is seeking a motivated and experienced Per Diem Occupational Therapist to join their team at a partnered assisted living facility in Westchester, NY. This role offers the opportunity to work independently while making a meaningful impact within a supportive, team-oriented memory care environment. The position involves providing 1:1 occupational therapy treatment, developing individualized treatment plans, and leading a weekly exercise class.

Requirements

Graduate of an accredited Occupational Therapy program
Licensed (or eligible) Occupational Therapist in New York State
CPR, AED, and First Aid certifications required
Prior clinical experience required
Candidates must be comfortable working independently

Nice To Haves

Experience with geriatric and/or memory care populations strongly preferred
Strong interpersonal and communication skills

Responsibilities

Provide 1:1 occupational therapy treatment to residents within the facility
Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
Deliver care in accordance with professional standards and the established plan of care
Lead a weekly group exercise class focused on mobility and functional wellness
Collaborate with facility staff and caregivers to support patient outcomes
Plan and implement therapy programs to improve or restore functional abilities, reduce limitations, and enhance quality of life
Perform and document initial evaluations, progress notes, and discharge summaries in a timely and accurate manner
Monitor patient progress and modify treatment plans as needed to achieve optimal results
Provide care with a strong understanding of geriatric and memory care populations
