About The Position

The PACE Occupational Therapist (OT) is a progressive, mission-driven role focused on improving the health and quality of life for older adults participating in the PACE Program. This position provides assessment, treatment, and interventions, and oversees optimizing function and honoring what matters most to PACE participants. As a member of the PACE interdisciplinary team (IDT), the role actively participates in the development and implementation of the IDT Plan of Care across the continuum.

Requirements

• A minimum of 3 years of experience in various healthcare settings is required, preferably in care management, geriatrics, chronic disease management, home health, or long-term care.
• 1 year of experience working with frail, older adults is required.
• A Bachelor’s degree from an accredited occupational therapy program and OT licensure is required.
• CPR certification (BLS or ACLS) is required.
• A valid driver's license is required.
• Ability to work effectively with individuals who have diverse health issues, concerns, and personalities.
• Proficient in accurately documenting case notes, assessments, contacts, and writing letters that may become part of legal records.
• Excellent organizational skills and experience with various computer software applications, including Word, Excel, PowerPoint, and Outlook, are essential.
• Capability to work independently with minimal supervision is required.
• Exceptional communication skills, both verbal and written.
• Strong active listening skills are also necessary.
• Must be legally authorized (for example, currently licensed, registered or certified if applicable) to practice in the State in which he or she performs the function or action.
• Only act within the scope of his or her authority to practice.
• Must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
• Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.
• Be medically cleared for communicable diseases before engaging in direct participant contact.
• Stooping, bending, sitting, standing, and moving between clinical areas for extended periods.
• May assist participants with mobility, including guiding, supporting, escorting, and occasional transfer assistance using proper body mechanics.
• Must be able to lift, push, pull, or carry up to 35 pounds occasionally and perform frequent repetitive motions such as typing and operating medical or office equipment.
• Must be able to speak clearly, understand verbal instructions, and interact professionally with participants, caregivers, and staff.

Responsibilities

• Provide Occupational Therapy treatment and interventions to PACE participants across the continuum, including in the home and/or community setting.
• Conduct comprehensive assessments of participants to determine their functional abilities and needs.
• Actively participate in the interdisciplinary team by offering clinical expertise in team planning, problem-solving, and processes.
• Develop and implement personalized care plans aimed at improving or maintaining participants’ independence and quality of life.
• Guide participants in therapeutic, creative, and self-care activities to enhance their functional abilities.
• Review participants’ progress and adapt treatment plans as needed.
• Provide training, guidance, and supervision to Occupational Therapy Assistants (COTA) or Personal Care Assistants, and participant/family members in carrying out the Functional Maintenance Program.
• Collaborate closely with other healthcare professionals, including physicians, nurse practitioners, physician associates, nurses, physical therapists, social workers, and other members of the interdisciplinary care team, to provide integrated care.
• Meet all administrative requirements, including attendance at team and staff meetings, and recording and tracking OT rehabilitation equipment.
• Visit participants in their homes or care settings as needed.
• Serve on various internal and external committees as required or designated.
• Provide all services in a manner consistent with State and Federal regulatory requirements.
• Participate in team meetings, caregiver groups, and special events as needed.
• Document in the electronic health record accurately, professionally, and in a timely manner.
• Foster a safe and supportive environment conducive to the professional development of healthcare professionals.
• Meet department and company performance and attendance expectations.
• Follow privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
• Perform other duties as assigned.
