You'll play a critical role in empowering patients and their families throughout the rehabilitation process.

Requirements

  • Must have graduated from an accredited school of occupational therapy.
  • Current state license as an Occupational Therapist.
  • Current CPR certification.
  • Six months experience in a long term care environment preferred.

Responsibilities

  • Evaluate referred patients to determine medical condition, functional limitations, rehabilitation potential, and therapy needs.
  • Develop and implement individualized treatment plans based on physician orders and patient goals.
  • Establish realistic, measurable, and achievable therapy objectives tailored to each patient's abilities and tolerance levels.
  • Provide skilled occupational therapy services and interventions to improve daily living skills, independence, and overall function.
  • Assess patient progress and modify treatment plans as needed to achieve optimal outcomes.
  • Screen residents and evaluate patients who may benefit from occupational therapy services.
  • Collaborate with physicians, nursing staff, rehabilitation professionals, and interdisciplinary team members to coordinate patient care.
  • Refer patients to Certified Occupational Therapy Assistants (COTAs) when appropriate.
  • Provide consultation, guidance, and clinical supervision to COTAs and Rehabilitation Aides.
  • Support the ongoing development of therapy staff by promoting best practices and clinical excellence.
  • Maintain accurate and timely documentation of patient evaluations, treatment plans, progress, and outcomes.
  • Adhere to Minimum Data Set (MDS) procedures and effective care planning processes.
  • Ensure compliance with all applicable state practice acts, OBRA regulations, and facility policies.
  • Communicate patient progress, status changes, and care needs to patients, families, and healthcare team members.
  • Build strong relationships with patients, families, and staff through compassionate and professional interactions.
  • Participate in utilization reviews, case management discussions, family conferences, and interdisciplinary meetings.
  • Contribute to a positive rehabilitation culture that promotes exceptional patient outcomes.
  • Maintain punctuality and reliable attendance for assigned shifts.
