About The Position

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health Occupational Therapist to provide individualized care to patients in their homes in west Hillsborough County, including Tampa, Lutz, Carrollwood, South Tampa, Westchase, and surrounding areas. Roles may also be available in east Hillsborough, Pasco, Pinellas, Hernando, and Manatee counties. The schedule is PRN, with 5–10 or more home health visits per week. Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support. The clinician provides skilled occupational therapy and coordinates with an interdisciplinary team to support holistic, patient-centered care in the home. Requirements

Active Florida occupational therapist (OT) license.
At least 1 year of experience as a licensed Occupational Therapist.
Strong assessment, critical thinking, and patient education skills.

Home health care experience is strongly preferred.

Responsibilities

Perform comprehensive patient assessments, including OASIS assessments at required intervals.
Develop, initiate, and revise individualized plans of care as patient needs evolve.
Deliver skilled occupational therapy services and treatments as outlined in the plan of care.
Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
Collaborate with the interdisciplinary team to ensure seamless, holistic care.
