About The Position

Therapy Partner Solutions (TPS), a therapist-owned and operated company since 2016, is dedicated to providing exceptional therapy services in home health care. As the provider of home health therapy services for Community Home Care, TPS is part of a strong, multidisciplinary team that includes physical therapists, occupational therapists, speech therapists, nurses, physicians, and other healthcare professionals. They focus on collaborative, coordinated, patient-centered care and support their clinicians through mentorship, advanced training, and educational series. Requirements

• Graduate (current or pending) from an accredited occupational therapy program.
• Active or pending state occupational therapy license.
• Comfort working independently in the home setting.
• New Graduates welcome to apply.

Responsibilities

• Work with a diverse and engaging caseload.
• Provide home health therapy services.
• Collaborate with a multidisciplinary team including physical therapists, occupational therapists, speech therapists, nurses, and physicians.
• Deliver collaborative, coordinated, patient-centered care.
• Support patients in the home healthcare setting.
• Participate in individualized clinical and professional development programs.
• Complete documentation training to encourage confidence and efficiency.
