About The Position

Talin is seeking a dedicated and compassionate Occupational Therapist to provide high-quality care in patients’ homes. The role focuses on improving the functional abilities and quality of life of people with disabilities, injuries, or age-related needs. The Occupational Therapist develops personalized care plans, assesses patients, and implements therapeutic interventions. The coverage area is Loudoun, with other areas available. Work is on the road.

Requirements

Degree in Occupational Therapy from an accredited program.
Valid state licensure as an Occupational Therapist.
Strong knowledge of physiology and occupational health principles.
Excellent communication skills for interacting with patients, families, and interdisciplinary teams.
Ability to develop effective care plans that address individual patient needs.
Compassionate nature and a strong commitment to patient care and rehabilitation outcomes.

Nice To Haves

Experience in case management and working with individuals with disabilities is preferred.
Familiarity with early intervention strategies is a plus.

Responsibilities

Conduct comprehensive evaluations of patients’ physical, emotional, and social needs.
Develop and implement individualized care plans based on each patient’s goals and capabilities.
Provide direct patient care through therapeutic activities that improve daily living skills and overall functionality.
Collaborate with healthcare professionals, including speech therapists and case managers, to support holistic care.
Educate patients and families about treatment options, progress, and ongoing support strategies.
Monitor patient progress and adjust treatment plans based on observed outcomes.
Maintain accurate documentation of assessments, treatment plans, and progress notes in compliance with policies.
