About The Position

This part-time Occupational Therapist position involves cross-covering multiple rehabilitation and transitional care units within Trillium Health Partners, including the Reactivation Care Centre, Humber RCC, Moir CCC, and McCall Centre. Trillium Health Partners is a large community-based teaching hospital serving diverse populations in Mississauga, West Toronto, and surrounding areas. The rehabilitation team provides patient-centered care, aiming to guide patients towards independence and reintegration into the community by improving physical, cognitive, and communication challenges resulting from injury or illness. Patient success is optimized through self-motivation, active participation, and family support.

Requirements

  • Registered and in good standing with the College of Occupational Therapy of Ontario
  • Certified in FIM assessment, or willingness to be certified within 6 months of hire
  • MoCA training certification, or willingness to obtain
  • Superior interpersonal and communication skills
  • Demonstrated effectiveness working independently as well as liaising with the interprofessional team
  • Satisfactory performance and attendance required

Nice To Haves

  • CAOT or OSOT member preferred
  • A breadth of experience, including but not limited to assessment and treatment of post-acute medically complex, orthopaedic, neurogenic, geriatric and vascular patients (including amputees)
  • Experience with acute medicine patients and transitions in care is an asset
  • Knowledge of community resources and services is an asset
  • Current certification in Basic Cardiac Life Support preferred
  • Demonstrated leadership within a interprofessional team is preferred

Responsibilities

  • Assessment of patients with functional impairment due to a variety of underlying medical conditions including but not limited to cardio-pulmonary, musculo-skeletal and neurological systems
  • Co-creating of goals with the patient and family
  • Planning and implementation of treatment plans to improve function and participation in activities of daily living
  • Participate in patient discharge planning
  • Document patient assessment and treatment in the health record
  • Collaboration with the interprofessional healthcare team, patients and their families to achieve patient goals and augment the treatment plan as needed
  • Providing education to the interprofessional team to support evidence-based care
  • Assigning to and supervision of support personnel
  • Involvement in clinical education of various professions, including preceptorship of occupational therapy students
