About The Position

Hartford HealthCare at Home, the largest provider of homecare services in Connecticut, is seeking an Occupational Therapist. This role involves evaluating patients' functional levels, developing and implementing individualized treatment plans, and promoting health and wellness. The therapist will work to improve patient independence, productivity, and overall well-being in various contexts. The position may involve working in different environments including Inpatient, Outpatient, Home Care, Skilled Nursing, or Assisted Living. The therapist will also delegate patient care responsibilities and provide clinical supervision.

Requirements

  • Bachelors of Science Degree in Occupational Therapy required.
  • Current State of CT Occupational Therapy license required.

Nice To Haves

  • Master’s Degree in Occupational Therapy or Doctor of Occupational Therapy preferred.
  • Previous experience as an Occupational Therapist preferred.

Responsibilities

  • Independently evaluate patients’ level of function and determine, develop, and implement individualized treatment goals and plans.
  • Determine treatment strategies and deliver Occupational Therapy services to promote health and wellness, prevent injury or disability, improve and develop self-dependency and maintenance, productivity, leisure, perceptual motor function, rest, mental, cognitive physical, sensory-perceptual, psychosocial developmental, and emotional wellbeing.
  • Delegate patient care responsibilities to Certified Occupational Therapy Assistant.
  • Perform thorough evaluation of patient’s level of function upon Physician referral.
  • Perform evaluations for upper extremity strength, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, perceptual-cognitive motor function, and functional abilities of daily living.
  • Recognize physiological, psychological, and age related changes in the patient.
  • Consult with other Health Care Team members as necessary.
  • Develop an individualized patient centered treatment plan of care.
  • Evaluate care plan on an on-going basis, revising as necessary in accordance with patient’s care needs.
  • Establish measurable goals with the patient/family based on appropriate patient care protocols and critical paths.
  • Notify Physician of any changes in status or progress.
  • Deliver therapeutic techniques that are appropriate to patient’s needs and medical status.
  • Implement, deliver and delegate patient care by utilizing appropriate interventions and clinical decision making.
  • Provide patient and caregiver education regarding treatment, precautions, and equipment use.
  • Complete all documentation including but not limited to: evaluation, care plan and goals, patient status and progress, visits, patient discharge summary, and patient care transition plan and summary.
  • Ensure timely and accurate documentation and coding of skilled and medically necessary services provided to the patient.
  • Participate in patient transition planning and/or patient discharge planning including recommendations to community resources.
  • Help to coordinate patient care with other care providers such as Rehab Therapists, Nurses, Social Workers, and other community resources.
  • Delegate patient care tasks and provide clinical supervision for Occupational Therapist students, Rehabilitation Aides and Certified Occupational Therapy Assistants.
  • Maintain a clean, neat work environment for patients and staff including adhering to infection control and safety precautions.
  • Restock supplies as necessary.
  • Maintain clinical proficiency, continuing education units (CEUs), annual health stream, and all education requirements as required by the State of Connecticut licensure.
  • Perform other related duties as required.
