About The Position

Valley Home Health Care is seeking an Occupational Therapist to meet with patients, discuss their treatment needs and goals, and create customized, comprehensive therapy plans. The therapist will coordinate with doctors and other members of patients’ care teams to arrange follow-up care, educate patients about relevant lifestyle changes, and provide a positive, affirming presence. The role involves working with several patients and different cases in a day.

Requirements

Hold the required state occupational therapy license.
Strong communication and interpersonal skills.
Excellent organizational skills.
Good attention to detail.
Ability to remain positive and compassionate while working with patients over time.

Responsibilities

Meet with patients from diverse backgrounds and discuss their health concerns and therapy needs compassionately.
Conduct evaluations to determine a starting point for each patient.
Create individualized therapy plans to help patients meet their mobility goals.
Work with other members of each patient’s care team to provide adequate follow-up care.
Maintain accurate records of patient therapy and progress.
Help educate patients about lifestyle changes they may need.
