About The Position

Nursing & Rehab At Home is a clinician-owned, family-run home health agency rooted in its local community. The Home Health Occupational Therapist provides care in patients’ homes, building relationships with patients and families and helping patients become more independent. The role offers autonomy with support from a Clinical Manager and care team, and involves collaboration with physicians, nurses, therapy assistants, and other clinicians. The position is described as part time or per diem.

Requirements

Graduate of an accredited occupational therapy program.
NBCOT certification.
Active California occupational therapy license.
Current CPR certification.
Reliable, insured vehicle and valid driver’s license.
Strong communication and organizational skills.

Nice To Haves

At least 1 year of occupational therapy experience preferred.

Responsibilities

Evaluate patients’ functional abilities and assess their homes for hazards or barriers to independent living.
Develop individualized treatment plans and goals and share them with referring physicians and the care team.
Teach or retrain patients in daily activities and train them to use adaptive equipment, prosthetics, and orthotics; fabricate splints when needed.
Manage assigned patient cases when requested, coordinating care from start through discharge.
Complete OASIS assessments as assigned.
Educate patients, families, and caregivers to support recovery.
Supervise certified occupational therapy assistants and, when therapy is the only skilled service, home health aides.
Connect patients and families with other home health services they may need.
Participate in case conferences and in-services.
