About The Position

This position bridges inpatient care and community rehabilitation, ensuring safe, effective transitions from hospital to home. You’ll work with patients recovering from surgery, acute illness, complex geriatric conditions, progressive disease, or functional decline - focusing on preventing readmissions, promoting recovery, and maximizing independence. At Spectrum, you’ll practice at the intersection of acute-care clinical reasoning and community-based service delivery. You’ll support clients recently discharged from hospital or following post-surgical pathways, backed by a collaborative interdisciplinary team and a flexible, responsive work environment. Requirements

  • Completion of a recognized Occupational Therapy Program and licensed with the College of Occupational Therapists of Ontario (COTO).
  • Valid Ontario driver’s license and reliable vehicle.

Nice To Haves

  • 3+ years of experience preferred (but not required) and ideally in community settings.
  • Experience with assessments, treatment planning, and working with diverse patient populations.

Responsibilities

  • Assess and Support Patients: Assist patients with overcoming physical, cognitive, and emotional barriers to maximize their independence in daily activities.
  • In-Home Assessments: Conduct comprehensive in-home assessments to evaluate the physical, cognitive, and psychosocial abilities of clients.
  • Collaborative Care Plans: Develop individualized care plans with direct input from clients and their families and to provide input and direction on the frequency and duration of needed OT interventions to meet client goals.
  • Interdisciplinary Collaboration: Work closely with physicians, nurses, social workers, and other professionals to ensure the best outcomes for clients.
  • Adaptive Equipment & Safety: Assess and train clients and families on the use of adaptive/assistive equipment, safety equipment, mobility aids, and environmental modifications.
