About The Position

We are currently seeking a compassionate and dedicated Occupational Therapist (OT) to join our growing home health team. Requirements

• Active California Occupational Therapist (OT) license
• Graduate of an ACOTE-accredited Occupational Therapy program
• Minimum 1 year of clinical experience
• Experience with EMR systems and electronic documentation
• Strong communication, organization, and time management skills
• Valid driver’s license, reliable transportation, and auto insurance

Nice To Haves

• Home health experience preferred

Responsibilities

• Provide in-home occupational therapy evaluations and reassessments
• Develop and implement personalized rehabilitation care plans
• Help patients improve activities of daily living (ADLs) and functional mobility
• Support recovery following injury, surgery, or hospitalization
• Promote home safety, fall prevention, and independence
• Collaborate with interdisciplinary teams including nurses, PTs, and physicians
• Educate patients and caregivers on therapeutic techniques and safety practices
• Maintain accurate documentation in compliance with Medicare and state regulations
