About The Position

Human Touch Home Health is looking for a compassionate and motivated Occupational Therapist (OT) to join our Washington, DC team Full-Time. We have patient opportunities throughout the District and are looking for an OT who wants the stability of a full-time position while still enjoying the independence and one-on-one patient care that comes with home health. This is your opportunity to build relationships, manage a consistent caseload, and see the real-life impact of occupational therapy in the patient's own environment.

Requirements

Active Washington, DC Occupational Therapist License Required
Graduate of an accredited Occupational Therapy program
Reliable transportation and ability to travel between patient homes
Strong clinical, communication, and organizational skills
Ability to independently manage a full-time home health caseload
Strong time-management skills

Nice To Haves

Home Health Experience Preferred, but Not Required
WellSky Experience is a Plus

Responsibilities

Complete occupational therapy evaluations in patients' homes
Develop individualized plans of care
Help patients regain independence with activities of daily living
Provide therapeutic interventions to improve strength, coordination, mobility, and functional ability
Address dressing, bathing, grooming, toileting, meal preparation, and other everyday activities
Assess home safety and recommend adaptive equipment or modifications
Educate patients, families, and caregivers
Monitor patient progress and adjust treatment plans as needed
Complete reassessments and discharge planning
Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary team
Complete accurate and timely documentation using WellSky
