Full-time Occupational Therapist (OT)
About The Position
Human Touch Home Health is looking for a compassionate Occupational Therapist (OT) to join our team Full-Time in the Fredericksburg area. Patient opportunities are available throughout Fredericksburg, Stafford, Falmouth, Spotsylvania, Massaponax, Spotsylvania Courthouse, Garrisonville, and surrounding communities. This full-time therapy role allows you to work outside of a clinic setting and directly impact a patient's daily life. Occupational therapy at home is where independence becomes real. You're not asking a patient to practice daily living skills in a simulated environment. You're helping them get dressed in their own bedroom, safely use their own bathroom, prepare food in their own kitchen, and navigate the home they want to remain in. You get to see the barriers. You get to help create the solutions. And you get to watch patients regain pieces of their independence that truly matter. That's OT with a Human Touch.
Requirements
- Active Virginia Occupational Therapist License Required
- Graduate of an accredited Occupational Therapy program
- Valid Driver's License and reliable transportation
- Strong clinical, communication, and organizational skills
- Comfortable managing a full-time home health caseload
Nice To Haves
- Home Health Experience Preferred, but Not Required
- WellSky Experience is a Plus
Responsibilities
- Complete occupational therapy evaluations in patients' homes
- Develop individualized plans of care
- Help patients regain independence with activities of daily living
- Provide therapeutic interventions to improve strength, coordination, mobility, and functional ability
- Address dressing, bathing, grooming, toileting, meal preparation, and other daily activities
- Assess home safety and recommend adaptive equipment or modifications
- Educate patients, families, and caregivers
- Monitor patient progress and adjust treatment plans as needed
- Complete reassessments and discharge planning
- Coordinate care with physicians, nurses, therapists, and other members of the care team
- Complete accurate and timely documentation using WellSky
