About The Position
Ultimate Therapy is seeking a part-time Occupational Therapist to provide skilled therapy services in a skilled nursing facility. This role focuses on helping residents improve daily function, safety, and independence.
Requirements
- Active Occupational Therapist license required

- SNF experience preferred
Responsibilities
- Complete evaluations and develop individualized treatment plans
- Provide therapy focused on ADLs, mobility, cognition, and functional independence
- Utilize adaptive techniques, equipment training, and environmental modifications
- Monitor progress and adjust interventions as needed
- Collaborate with interdisciplinary team members and communicate with families
- Maintain accurate and timely documentation
