We are seeking a compassionate and motivated Occupational Therapist (OT) to join our growing home health team. Requirements

Current Occupational Therapist (OT) license in good standing
Strong clinical judgment and patient assessment skills
Ability to work independently in a home health setting
Excellent communication and teamwork skills
Compassionate, patient-centered approach to care
Valid Occupational Therapy License
CPR certification
Valid Driver’s License & Auto Insurance

Nice To Haves

Experience in home health is a plus, but not required
HomeCare HomeBase experience preferred
1 year of occupational therapy experience preferred

Responsibilities

Conduct comprehensive clinical evaluations to determine patient needs and therapy appropriateness
Develop, implement, and update individualized Plans of Care
Provide skilled OT interventions to improve: Strength, coordination, and range of motion Functional mobility and safety ADL performance (self-care, dressing, bathing, eating, etc.)
Assess cognitive, sensory-motor, psychosocial, and environmental factors affecting function
Fabricate and maintain splints and adaptive equipment
Recommend home/environmental modifications to promote safety and independence
Participate in interdisciplinary care planning (IDG) to ensure coordinated patient outcomes
Maintain accurate, timely clinical documentation (evaluations, progress notes, orders)
Follow infection control and safety standards in all patient care activities
