About The Position
Occupational Therapist (OT) – Full-Time Facility: Gateway Post Acute Location: Porterville, CA Employment Type: PRN / Per Diem Pay Range: $60.00 – $65.00 per hour (BOE) Empower Recovery. Inspire Independence. Gateway Post Acute is a premier skilled nursing and post-acute rehabilitation facility located in Porterville, CA. Known for our compassionate care, clean environment, and supportive team culture, we are currently seeking a PRN Occupational Therapist (OT) to join our rehabilitation team. This is an excellent opportunity for a motivated therapist who enjoys flexibility while making a meaningful impact on patients’ functional independence and quality of life.
Requirements
- Bachelor’s degree in Occupational Therapy required
- Current, active California Occupational Therapist license in good standing
- Current CPR/BLS certification
- Strong clinical judgment and patient-centered approach
- Proficient in electronic documentation systems (PointClickCare and/or Casamba preferred)
- Excellent communication, time-management, and organizational skills
- Ability to work independently and as part of a collaborative team
Nice To Haves
- Master's or Doctorate degree preferred
- Skilled Nursing Facility or post-acute experience preferred
Responsibilities
- Evaluate patients and establish individualized occupational therapy treatment plans
- Treat patients in accordance with physician orders and approved plans of care
- Document evaluations, daily treatment notes, progress notes, recertifications, and discharge summaries per regulatory and payer requirements
- Educate patients and families on adaptive techniques, safety, and functional independence
- Collaborate with nursing, therapy, and interdisciplinary teams to support optimal patient outcomes
- Participate in patient care conferences, rehab meetings, and discharge planning as needed
- Recommend and facilitate ordering of adaptive equipment and durable medical equipment (DME)
- Maintain safe, clean, and organized treatment areas
- Report equipment issues to ensure proper operation and safety
- Maintain patient confidentiality and uphold all resident rights and privacy standards
