About The Position

Health Link Home Health & Hospice is seeking a compassionate, organized, and team-oriented Occupational Therapist (OT) for both Full Time (SF area) and Part Time (Peninsula area) positions. With over 20 years of service in the greater Bay Area, Health Link focuses on clinical quality, strong systems, and a collaborative culture, believing that excellent patient care begins with supporting their clinicians. The OT will be assigned to a dedicated territory within either San Francisco or San Mateo County to foster familiarity with patients and the local area. This role emphasizes providing skilled occupational therapy services, developing individualized care plans, collaborating with healthcare professionals, and maintaining accurate records using AI-supported tools.

Requirements

Current, active California Occupational Therapist license (required)
Minimum of 1 year of home health experience strongly preferred
Valid California driver's license
Strong organizational skills, clinical judgment, and a compassionate communication style

Responsibilities

Provide skilled occupational therapy services focused on ADLs, adaptive equipment, and functional independence
Develop, implement, and manage individualized plans of care based on physician orders
Report changes in patient condition and collaborate with physicians, case managers, and the Director of Nursing
Maintain accurate, timely clinical records using AI-supported documentation tools
