About The Position

Health Link Home Health & Hospice is seeking a compassionate, organized, and team-oriented Occupational Therapist (OT) for Full Time positions in the SF area and Part Time positions in the Peninsula area. For over 20 years, Health Link has served the greater Bay Area with a focus on clinical quality, strong systems, and a collaborative culture. We are hiring Occupational Therapists for Peninsula teams. You will be assigned to one dedicated territory — San Mateo County — so you can build familiarity with your patients and your area without being stretched across both. The SF territory covers neighborhoods like the Sunset, Richmond, and Mission; the Peninsula territory runs from Daly City down through Redwood City. We utilize AI-assisted charting to reduce documentation time and provide real-time clinical support from experienced case managers and a clinical team.

Requirements

Current, active California Occupational Therapist license (required)
Minimum of 1 year of home health experience strongly preferred
Valid California driver's license
Strong organizational skills, clinical judgment, and a compassionate communication style

Responsibilities

Provide skilled occupational therapy services focused on ADLs, adaptive equipment, and functional independence
Develop, implement, and manage individualized plans of care based on physician orders
Report changes in patient condition and collaborate with physicians, case managers, and the Director of Nursing
Maintain accurate, timely clinical records using AI-supported documentation tools
