About The Position

FOX Rehabilitation is expanding in New Iberia, LA, and is seeking dedicated, licensed Occupational Therapists to provide exceptional one-on-one care to older adults in their homes or within Senior Living Communities. The role focuses on improving patient mobility, independence, and quality of life through the Geriatric House Calls™ model. This position is situated in one of our Senior Living Community partners and offers autonomy, flexibility, and the opportunity to manage one's own treatment schedule and build strong patient relationships. Clinicians may also integrate telehealth and virtual care for increased flexibility and patient support. This is a high-impact OT role aimed at positively influencing the lives of the communities served.

Requirements

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Occupational Therapy program
• Basic computer and EMR proficiency
• Current CPR certification
• Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE when required

Responsibilities

• Provide one-on-one Occupational therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility
