About The Position
FOX Rehabilitation is hiring licensed Occupational Therapists to provide one-on-one care to older adults in White Plains, NY, and surrounding communities. Care is delivered in patients’ homes and senior living settings, including independent living, assisted living, and memory care. Clinicians use the Geriatric House Calls™ model to help patients improve mobility, independence, wellness, and quality of life. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Clinicians may also incorporate telehealth and other virtual care into their schedules. FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and a mission to abolish ageism. The position involves collaboration with an interdisciplinary team and education for patients, caregivers, and senior living staff. New graduates are eligible for FOX’s Emerging Professionals Mentor Program. FOX offers full-time, part-time, and PRN/FOX Flex pathways; compensation and benefits vary by employment type and eligibility.
Requirements
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
- Document in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
