About The Position
This position is for an Occupational Therapist (OT) working in a Skilled Nursing Facility (SNF). The role involves evaluating and treating patients with functional, cognitive, and ADL limitations. The therapist will develop individualized treatment plans, document patient progress, and collaborate with patients, families, and the interdisciplinary team. The setting is an 80-bed Skilled Nursing Facility.
Requirements
- Active Colorado Occupational Therapist (OT) license required.
- Graduate of an accredited Occupational Therapy program required.
- Proficiency with therapeutic activities, exercises, ADL retraining, mobility, and adaptive equipment.
- Strong discharge planning, patient/family education, and interdisciplinary collaboration skills.
- Comfortable working in an 80-bed Skilled Nursing Facility (SNF).
Nice To Haves
- SNF, LTC, rehabilitation, or geriatric experience preferred.
- Experience evaluating and treating patients with functional, cognitive, and ADL limitations.
