About The Position

We have an exciting opportunity to join our team as a Occupational Therapist - Outpatient Ortho - Patchogue. In this role, the successful candidate will provide appropriate therapy to help patients improve their ability to perform activities of daily living and to achieve optimal level of self-care and productivity. Uses clinical expertise to plan the delivery of safe, therapeutic, and age appropriate care through examination, evaluation, diagnosis and physical intervention.

Requirements

  • Bachelor's Degree Occupational Therapy (Accreditation Council of Occupational Therapy Education (ACOTE) approved OT program).
  • Current and valid NYS Occupational Therapy license and registration or limited permit.
  • Current and valid BLS or CPR certification from the American Heart Association - Inpatient facility only.
  • Excellent communication (both oral and written) skills.
  • Basic computer skills.
  • Qualified candidates must be able to effectively communicate with all levels of the organization.

Nice To Haves

  • Master's Degree.
  • Current and valid BLS or CPR certification from the American Heart Association - Outpatient facility only.
  • 1- 2 years Hospital (Inpatient) or Orthopedic (Outpatient) Rehabilitation setting.
  • Experience in Neuro - Outpatient facility only.

Responsibilities

  • Maintains accurate, complete and timely documentation reflecting the examination, evaluation, diagnosis, prognosis, plan of care, education and interventions.
  • Interacts and collaborates with other members of the healthcare team to optimize patient outcomes and health care goals. Provides on-site clinical supervision to Occupational Therapist Assistants and Rehabilitation Aides.
  • Evaluates patient progress toward functional goals, and will modify the the plan of care accordingly.
  • Provides patient, family, and clinician education regarding the patient's functional status and plan of care.
  • Performs other duties as assigned
  • Performs an objective functional evaluation including appropriate tests and measures and establishes a plan of care with measurable goals.
  • Implements appropriate interventions treatment as indicated by the patient's functional status, diagnosis, and age in conjunction with the plan of care.
