About The Position
This is an excellent opportunity for therapists who enjoy one-on-one patient care, flexible scheduling, and helping patients maximize independence in their own homes. @Home Therapy is seeking a licensed Occupational Therapist (OT) to provide home-based occupational therapy services to adult and geriatric patients throughout North Queens.
Requirements
- Current New York State Occupational Therapist license.
- Strong communication and clinical reasoning skills.
- Comfortable working independently and using EMR documentation.
Nice To Haves
- Home health experience is a plus but not required.
Responsibilities
- Perform occupational therapy evaluations in the home.
- Assess ADLs, IADLs, upper extremity function, cognition, coordination, home safety, and functional independence.
- Develop individualized treatment plans to improve daily function and quality of life.
- Train patients and caregivers in adaptive equipment, energy conservation, fall prevention, and safe performance of everyday activities.
- Monitor patient progress and modify treatment plans as needed.
- Complete timely EMR documentation and collaborate with physicians and the interdisciplinary care team.
