Primary Care Children’s Services will establish four Primary Care Paediatric Network Teams aligned to two Community Healthcare Networks, in line with the National Paediatric Model of Care (2016). These teams will operate within Primary Care Services as part of a single integrated service model and will work alongside existing Primary Care clinicians to support children and young people whose needs require a coordinated multidisciplinary response. The Paediatric Network Teams are not a separate service rather, they are an enhancement of the existing Primary Care Children Services, bringing together Occupational Therapists and other Primary Care professionals to work collaboratively around the needs of children and families. Children accessing Primary Care may continue to receive care from their local Primary Care clinicians, with the Paediatric Network Teams providing additional multidisciplinary assessment, intervention, consultation and care coordination where required. Responsibility for care will remain shared across the wider Primary Care team, ensuring a coordinated and seamless experience for children and families. The teams will provide: Multidisciplinary assessment, including participation in diagnostic pathways such as autism assessment. Coordinated intervention and care planning for children and young people with non-complex disabilities and developmental needs requiring a multidisciplinary response within Primary Care. Post-diagnostic supports, therapeutic programmes and education groups for children, young people and families accessing Primary Care services. Consultation, joint working and shared care with existing Primary Care clinicians to ensure integrated support for children and families. Integrated working with CDNTs, CAMHS and other services through defined care pathways and shared care arrangements. The overall aim is to ensure timely access to safe, effective and coordinated care for all children and young people in HSE Mid West. Children will be supported through a single Primary Care team approach, with existing Primary Care clinicians and the Primary Care Paediatric Network Teams working together to deliver the right support, at the right time, within the local Primary Care service.
