About The Position

Patriot Homecare is hiring a PRN Occupational Therapist Assistant to provide homecare services in Trumbull County. Under the direction and supervision of an Occupational Therapist, the assistant provides delegated patient care, helps with evaluations and rehabilitative care planning, and supports patients and families with exercise programs. The role includes participating in clinical and daily progress documentation. The position is paid at a competitive per diem visit rate. Patriot Homecare is veteran-owned and family-operated.

Requirements

Graduate from an Occupational Therapy Assistant education program accredited by the Accreditation Council for Occupational Therapy Education of the American Occupational Therapy Association, Inc.
Hold a license issued by the State of Ohio.
Have at least one year of institutional experience.
Complete a criminal background check.
Hold current CPR (BLS) certification.
Be able to drive and access patients’ homes, which may not be routinely wheelchair accessible.
Have sufficient hearing, eyesight, and physical dexterity to assess patients’ conditions and provide patient care.
Be able to bend, stoop, reach, and move patient weight up to 250 pounds on occasion, and lift or carry up to 30 pounds.

Responsibilities

Provide patient care services delegated by the Occupational Therapist and work under the therapist’s direction.
Participate in preparing clinical and daily progress notes.
Instruct patients’ families and assist in teaching exercise programs.
Under OT supervision, assist with patient evaluation, development of the rehabilitative plan of care, and periodic re-evaluation as indicated.
Perform other duties as assigned.
