About The Position

Our therapists work with older adults and geriatric patients in a home-based setting while receiving strong administrative and clinical support from an experienced team. There will also be patients at a facility in Port Jervis. Coverage zones are designed to minimize travel and support efficient scheduling.

Requirements

  • Active New York Occupational Therapist Assistant license
  • Degree from an accredited Occupational Therapy Assistant program
  • Reliable transportation for home visits
  • Strong communication and a patient-centered mindset
  • New graduates encouraged to apply

Responsibilities

  • Treat patients in the home health setting
  • Implement individualized plans of care
  • Provide therapeutic exercise, manual therapy, and functional training
  • Educate patients and caregivers
  • Complete documentation in the EMR system (HelloNote experience is a plus)
