About The Position

Extended Home Care is seeking a compassionate and dedicated Certified Occupational Therapy Assistant (COTA) to provide home-based occupational therapy services to pediatric patients with Intellectual and Developmental Disabilities (IDD) throughout New York City. This is an excellent opportunity for clinicians looking for a flexible schedule, with a strong preference for candidates who are available to see patients after 3:00 PM, when children are home from school. The role involves working with children and families in a rewarding home-based setting, with opportunities available throughout all five NYC boroughs.

Requirements

  • Graduate of an accredited Occupational Therapy Assistant program.
  • Current New York State COTA license in good standing.
  • Strong communication, organizational, and clinical skills.
  • Ability to work independently in a home care setting.
  • Reliable transportation for patient visits.
  • We are specifically seeking clinicians who can provide visits after 3:00 PM, as the majority of our pediatric patients are available following school hours.

Nice To Haves

  • Minimum of two (2) years of occupational therapy experience preferred.
  • Pediatric and/or IDD experience strongly preferred.

Responsibilities

  • Provide occupational therapy services under the direction of the supervising Occupational Therapist and physician's plan of care.
  • Implement individualized treatment plans designed to improve functional skills, independence, sensory integration, motor development, and participation in daily activities.
  • Work with pediatric patients diagnosed with intellectual and developmental disabilities and other developmental challenges.
  • Educate parents, caregivers, and family members on therapeutic techniques, home exercise programs, and strategies to support progress between visits.
  • Monitor patient progress and communicate changes to the supervising therapist and interdisciplinary care team.
  • Maintain timely and accurate clinical documentation.
  • Participate in case conferences and care planning meetings as needed.
  • Collaborate with physicians, therapists, nurses, and other healthcare professionals to support positive patient outcomes.
  • Adhere to agency policies, compliance standards, and quality assurance initiatives.
