As an Occupational Therapist, you will assess and provide consultation, recommendations, and interventions to a diverse client base in their homes, utilizing evidence-based practices. You will ensure coordinated care delivery through effective communication with the client care team, problem-solve collaboratively to enhance client independence, and uphold CRS's core values. This role also involves providing advice, mentoring, consultation, and expertise in Occupational Therapy, conducting joint visits for performance reviews, and supporting colleagues facing clinical challenges. You will also be responsible for the clinical orientation of new team members and those transitioning between clinical areas, mentoring new graduates, and identifying/participating in best practice initiatives. Regular meetings with the Supervisor, Rehab Best Practice, are expected, along with acting as a resource for professional standards, policies, and procedures.

Requirements

  • Master’s Degree or equivalent in Occupational Therapy.
  • Membership in good standing with the College of Occupational Therapists of Ontario.
  • Minimum of 3 years’ experience in a Health Profession.
  • Minimum of 2 years in the community health care sector.
  • Teaching or preceptor experience.
  • Valid Driver’s License with access to a vehicle.
  • Compassionate and empathetic.
  • Self-driven with ability to work independently.
  • Skilled in caseload management.
  • Skilled at electronic documentation.
  • Able to provide virtual care.

Responsibilities

  • Assessing and providing consultation, recommendations, and interventions to a diverse group of clients in their homes using evidence-based best practice.
  • Ensuring coordinated delivery of care through ongoing communication with the client care team regarding plans and schedules for interventions.
  • Problem-solving in collaboration with the client and care team to create opportunities for more independence.
  • Promoting CRS’s core beliefs and values.
  • Providing advice, mentoring, consultation, and expertise related to the Occupational Therapy profession.
  • Providing joint visits with other Occupational Therapists and Service Providers to give input for performance reviews.
  • Supporting Occupational Therapists and Service Providers who face clinically challenging situations.
  • Providing clinical orientation for new Occupational Therapists and Service Providers or those changing clinical areas.
  • Mentoring groups for new graduates or those new to the community setting.
  • Identifying, promoting, and participating in best practice initiatives reflective of the requirements of the evolving health care system.
  • Meeting regularly with the Supervisor, Rehab Best Practice.
  • Acting as a resource in the implementation and interpretation of discipline-specific professional standards of practice.
  • Contributing to the development of standards of practice, clinical policies, and procedures relevant to the profession.
  • Assessing and implementing occupational therapy interventions in accordance with professional practice guidelines and standards established by VHA and the perspective regulatory college.
  • Implementing client interventions based on assessment information, client input and in conjunction with the client’s integrated care plan.
  • Ensuring that services provided are best practice, evidence-based and evaluated through the routine use of valid and reliable outcome measured.
  • Documenting all assessment findings, treatment plans, progress and discharge summaries of interventions as required by the profession, referral source and the organization.
  • Supporting safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
  • Participating in a weekend on-call roster within the geographical region to provide services based on client needs.
