The Occupational Health Case Manager serves as an educator and resource to employers and injured employees, providing health education to enhance understanding of conditions, treatments, and procedures, and interpreting care plans and medical results. This role utilizes nursing assessment and clinical reasoning skills to develop patient care plans that facilitate recovery and return to work. The Case Manager collaborates with multidisciplinary teams, coordinates care across the continuum, and supports patients and families by addressing psychosocial needs, advocating for care, and identifying barriers. Regular collaboration with providers ensures timely modifications to care plans and facilitates communication among all involved parties. The position requires the ability to assess and interpret patient data based on age-specific needs and to perform other related duties as assigned.
