The Nutritionist delivers medical nutrition therapy for both outpatients and, on occasion, inpatients, ensuring high-quality, patient-centered care. Lead diabetes education initiatives, including one-on-one counseling, group classes, and management of patients using advanced monitoring tools such as continuous glucose monitors and blood glucose monitors. Design and implement meal programs for adolescent wellness camps, incorporating nutrition education and promoting healthy lifestyle practices. The Nutritionist provides on-demand diabetes education for walk-in patients and collaborate with internal medicine and family medicine teams for seamless care. Maintain accurate and thorough clinical documentation in alignment with established standards and guidelines. Collaborate with an interdisciplinary diabetes education team, including certified educators, pharmacists, nurses, and technicians, to optimize patient outcomes. Partner with outpatient clinic teams to deliver continuity of care, addressing same-day patients. The Nutritionist performs both outpatient and inpatient public health nutrition services, including individual consultations and group education sessions.
