The primary purpose of this role is to assess the nutritional status of prenatal patients and develop a plan of care that will meet their nutritional needs and result in an optimal pregnancy outcome. This includes certifying patients in WIC if eligibility criteria are met, documenting assessments and care plans in the Crossroads system, and assigning food packages as needed. The role involves assessing nutritional status by collecting and evaluating anthropometric, biochemical, clinical, dietary, and eco-social data. An individualized nutrition care plan will be developed based on assessment findings, and patients will be assisted in understanding and implementing the plan through counseling and educational materials. Ongoing follow-up is required to monitor progress, ensure compliance, and adjust the care plan as needed. Therapeutic nutrition care plans will be developed for nutritionally high-risk prenatal patients, considering factors such as adolescence, diabetes, hypertension, obesity, anemia, renal disease, irregular weight gain patterns, and substance use. All assessments, care plans, interventions, and patient responses will be documented in the electronic medical record. The position also involves participating in monthly WIC and maternity staff meetings to coordinate nutrition procedures, goals, and patient care plans with the interdisciplinary team and presenting updated nutrition information. In the event of a disaster, assistance may be required. Other duties may be assigned by the Supervisor, Deputy Director, or Health Director.
