About The Position

The main function of this position is to provide direct and indirect care under supervision of the nurse to a selected group of residents. This role can be in skilled care department or assisted living department. The minimum education, experience and qualifications for this position are: current certification as a nursing assistant with the Illinois Department of Public Health; able to speak, read, write and comprehend English skills needed to use medical terminology on the job; ability to do simple arithmetic; good communication skills; interest in working with older adults; and general knowledge of the needs of geriatric residents. Requirements

  • current certification as a nursing assistant with the Illinois Department of Public Health
  • able to speak, read, write and comprehend English skills needed to use medical terminology on the job
  • ability to do simple arithmetic
  • good communication skills
  • interest in working with older adults
  • general knowledge of the needs of geriatric residents
  • Professional nurse with current license in the state of Illinois
  • Clinical background in the care of older adult patients

Nice To Haves

  • Baccalaureate degree preferred
  • Previous supervisory experience preferred
  • Long-term care experience preferred but not required

Responsibilities

  • Supports, interprets, and implements Presbyterian Livings’ policies.
  • Works collaboratively with team to support Presbyterian Livings’ person centered care and Operation Excellence programs.
  • Insures adherences to all State and City regulations pertaining to healthcare.
  • Participates the inter-disciplinary service plan conferences.
  • Supervises and directs the nursing staff, Nursing Assistants and any other nursing personnel assigned to healthcare.
  • Assists in orienting and evaluating new nursing personnel and temporary personnel on assigned units.
  • Assists with performance evaluations of staff.
  • Counsels staff as needed and recommends disciplinary action to Director of Nursing or designee and/or initiates disciplinary action personally when the situation warrants.
  • Maintain documentation as needed.
  • Supports the CMS and Presbyterian Living Infection Prevention initiatives as needed.
  • Coordinate resident medical appt. with physicians, transportation and staff.
  • Other duties assigned
